Pre existing conditions if it's a private policy. Sleep disorders, snoring, impotency, fertility, weight loss surgery, mental health etc to mention just a few.

Cosmetic works if not warranted for medically, dentistry, some maternal cover might not be included etc but in my opinion there is no such thing as a "typical health insurance policy". It all depends on the insurance company, the network level you enter into, the company you or your husband work for in terms of the medical cover you or he negotiate....
