To learn how RF & Ultrasound treatment works to achieve what many consider to be nothing short of miraculous results, we spoke to Dr Allen Rezai, Lead Consultant Plastic & Reconstructive Surgeon at Elite Plastic & Cosmetic Surgery Group of Dubai Healthcare City, who explains “Whilst not being a replacement for a surgical facelift or neck lift, Radio Frequency combined with Ultrasound can provide a very effective alternative for those not seeking a surgical solution, or as a supplemental treatment following facial surgery. According to Dr Rezai, frequently treated areas include around the eyes, neck, lower face and jaw line, where it can lift sagging jowls and enhance jaw line definition. “Treating the mid and lower face and neck is very popular because both women and men believe that those areas make them look tired or older,” explains Dr Rezai. “Additionally, sagging along the jawline and neck occurs in many people who lose mid-face and central cheek fat volume. So even if a person has been careful and protected themselves from the sun over the years, they can still often end up with age-related fat volume loss in the face.”
 
Dr Rezai explains that “The Exilis Elite functions by combining Ultrasound with Radio Frequency (RF) to deliver heat deeply into the skin, resulting not only in skin tightening but also fat loss and collagen remodeling. In collagen remodeling the older elongated strands of collagen are denatured by the RF energy. During the body’s own repair process these collagen networks are then stimulated, strengthened and aligned into a more youthful configuration that improves both the skin laxity and its texture, at once tightening and rejuvenating the areas being treated. RF also improves blood circulation, which in turn increases lymphatic drainage which helps the body to excrete the excess fat safely and effectively.”

According to Dr Rezai, a unique feature of Exilis Elite is its employment of a proprietary technology known as Energy Flow Control (EFC) which helps deliver maximum thermal effect to tissue in the shortest time whilst ensuring safety, comfort and control. “In aesthetic treatments,” explains Dr Rezai, “we need to balance using high energy levels without compromising the patient's comfort. The RF system we use automatically eliminates energy peaks, ensuring there is no sudden surge, resulting in optimal skin tightening and fat reduction. It also includes an advanced cooling system that provides greater control for accurate tissue heating. Cooler surface temperatures help the RF and Ultrasound energies go deeper,” explains Dr Rezai. “In this way, we can precisely target the tissue we want in order to tighten skin and reduce fat.”
 
Dr Rezai is also impressed with what is known as the Impedance Intelligence feature of the Exilis Elite. “I believe that this not only provides greater safety but also a more uniform energy distribution,” he says. “One of the problems with some less advanced RF systems is that when you go over certain areas, such as the mandible, jawline or forehead, you encounter varying degrees of electrical resistance, known as impedance, and higher levels of impedance mean that overheating and discomfort can occur for the patient, sometimes even burning. Furthermore soft tissue is more likely to be under-treated. The Impedance Intelligence feature allows the delivery of optimal controlled energy that eliminates hot spots and regions that are under-treated,” he continues. “In this way we can achieve a level of uniformity not possible with other RF devices. It is not just a question of safety but also one of effectiveness and quality of result,” says Dr Rezai.

Who can benefit from RF Facial Rejuvenation treatment? Dr Rezai explains that the treatment is suitable for patients of a wide age range, typically women and men from 25 to 70+ years who have lax skin and moderate to mild fat deposits, and who either don't want or are not yet ideal candidates for surgical solutions, or who are unable to undergo surgery due to underlying medical conditions. The main exclusions to those suitable for the treatment are women who are pregnant or nursing, and those fitted with a defibrillator or pacemaker or have any type of metal implant.
 
As to the treatment itself, Dr Rezai explains that “RF & Ultrasound treatment is not a single-session treatment and the number and length of each session will depend upon the area to be treated and the individual characteristics, cosmetic concerns and goals of each patient. But typically for face a course of 3-6 treatments with an interval of 7-14 days is required. Although patients usually notice a difference following the first session, the results keep improving over a period of 2-6 months, especially after skin tightening, as the collagen remodeling process occurs gradually. Maintenance treatments can also be carried out as and when needed.”
