You can get a standard labor contract at immigration when doing the visa. The from from immigration is the only one we have ever used.
Edit to add that they may also require that you have applied for her national id as well. I know we had to show them the receipt for the id card when my DH applied for our new maid a few months ago.

Hi,
our maid to be is here on an entry permit, we have already done the medical test in Ad and got the result and now we want to change it into a residence visa.
But where do we get the labour contract and the form from? Does my hubby just need to go to the Immigration office in AD with the papers, take our maid along and everything will be dealt with there?
Any advice if there is something else we need to think of? sorry, but all the information I found on the internet states the procedure in Dubai and Dubai Immigration told me, it could be differetnt in AD !?
