I guess you could get the current printed one, and then ask her to do a covering letter explaining the real dates and the instant printing system.

It was for some time back. My Dr tells me the sick leave certificates are issued by DED then and there and can not be backdated and when I ask her for a letter headed letter to document my genuine illness, which she had seen me for, she said 'No'! I need a certificate/report to say I was ill for the days I was off (2 days) and I am willing to pay for it too. What else can I do?
