10 month old severly constipated and now has fissure tears! HELP!

My dd Malak is now 10 months old, she had her open heart surgery 7 weeks ago. She has been on a lot of medications, but now we have weaned her off all except lasix (diuretic). For the last 3 weeks she has been suffering from severe constipation and I am at my wits end on what to do to get her regular!

I took her to the pead last weekend and she said to change her formula and only give her foods that help with relieving constipation, like prunes, pears, plums, apricots, etc...... and to have her sip on water from a sippy cup through out the day. Well for a week we have followed her advise but NO improvement to her constipation! She now has a cpl of hemorroids and a few little fissure tears!! She is in agony when it is time to POO!

I resorted to giving her duphalac to help her out and it did work after two doses over two days. But today we are back to constipation UGH. I dont want to keep giving her the laxative, I am afraid she would become dependent on it?????

Because of all her surgeries and hospital stays, she is not crawling, pulling up etc... so she is not very active yet. Maybe this could be one reason? Please give me advise on what has worked for your babies who have been constipated! I know the Lasix can play a part in the constipation, but her dose was cut in half last week when she visited her cardiologist. Also, I gave her a warm bath today which is the first one since her surgery as we were not allowed to get her incision very wet for the first 6 to 7 weeks. She had just been getting sponge baths until today. Hopefully the warm baths will help???

Well we have finally solved the problem for now! The Lactulose, diet change, etc..... was not working! She was too impacted. My cardiologist in Boston said she needs to be takng Mirilax daily in her morning bottle. Just so happens I had an unopened bottle that I brought with me from the states, just didnt think I could give it to a baby. Anyway, I called her cardiologist and her Pediatrician here and they both agreed Mirilax is completely safe and will deimpact. We started the Mirilax 5 days ago and it is working like a charm! She is such a happy baby now! It is a wonderful thing this Mirilax and it is very gentle!!! It is not a stimulant and you cannot become dependent on it! She only takes 1 tsp a day and she is VERY regular now, without straining and without having diarrhea! If any of your LO's become severly constipated the only place you can buy Mirilax from, is American Hospital :) ALSO, they have now taken her off Lasix and she is now medication free for the time being! Her eyes seem a bit puffy to me but she is pee peeing GREAT and has no other congestive heart failure symptoms. Keep fingers crossed she is able to remain med free for a while! (well other than Mirilax for now)

Oops sorry forgot to say - about the fissure tears - I used remicaine jelly with mine - it numbs the area temporarily in order that the little one can pass stools without pain(numbs the area). Don't know what you have been "lubricating" with.

What really worried me about this scenario is that you said your child had a diuretic - and is now constipated. She needs WATER in her system, the diuretic is making her expell all the liquid and looks like she is dehydrated, hence the constipation! I am not a doctor but my gut tells me SHE NEEDS TO REHYDRATE. Try talking to her doc and see if he would recommend Pedialyte and please give her plenty PLENTY PLENTY of liquids as well as lots of fiber-rich foods! ( do not give her gatorade or any of these adult rehydrating drinks!) WATER WATER WATER. Hope this helps. Keep us posted. Please don't do this! The reason she is on a diuretic is to keep the blood volume down in order not to strain the heart. I am sure your Dr gave you a fluid restriction diet so you will not take this advice. Lactulose syrup is very safe even for small babies and widely available. I would go this route if your cardiologist agrees. ps ETA I wouldn't worry about her becoming dependant on the duphalac if you don't try the lactulose (non addictive) since you would only use it for a few weeks. Think I'd be more concerned about the alternative (unhappy uncomfortable baby and mum) another ps Have to say well done for coming through what must have been a really stressful heartbreaking time! <em>edited by Aryanwynn on 26/05/2011</em>

Here's what worked for me, oatmeal with pitted prunes. Dice the prunes in small bits and mix it with oatmeal AND do not forget to give your child plenty PLENTY of WATER!!!! That is essencial as no matter how much fiber you ingest, it is the water that makes it "go"!!!

Sounds like your wee soul has been through the wringer. My Son suffered a lot when he was small and I swear over ripe papaya worked every time. xxx

when i had my first baby he was born without a bowel so we had loads of probs down in the bottom area. at one point he had to have a camera put into his genitals so they could check things there, and then afterwards obviously he didnt want to go to the toilet, i remember making him sit on the toilet for a long time trying to get him to go and he wouldnt, in the end we just tried and tried and tried eventually he started but then the problem was it was very painful and so it was a matter that the more he went the easier it got but its very difficult as you do have to sort of force them etc. but it wasnt easy. i would suggest to water everything down. if the bottom is sore then i used metanium which i found was the best cream ever, one application and it would get rid of nappy rash, as that would be a cause why you wouldnt want to go to the toilet.

Try removing ALL starchy foods from her diet (wheat,oats, potoatoes, rice, banana, sweet potato, corn, etc) and give her lots of fruits. Pear and apple juice has more of a laxative effect than the fruit itself. Try and give her plenty of purees (as the water content is higher). You can also rub a little lubricant around her bottom to help with the fissures. The laxatives are a good idea short term as when it is painful to poo kids can develop a habit of holding in the poo which unfortunately leads to more consipation, so you need to deal with the pain first before worrying about habit (but yes not good long term) If you can massage her tummy area it could help a bit too.
