My LO's just fallen below his previous centile on WHO growth charts... What to do?

So my little man is always going to be little - he's never going to top the scales. But having just weighed him today, form his previous weight at 2 mother (corrected) he was sitting on the 2nd percetile and now he's just dipped below the line (just)... so I go see his pead? Do I leave it till his next appointment (in one month)? Surely there's very little else i can do???

He's bf and still takes 7 feeds in a 24 hour period. I cluster feed in the afternoon and feed on demand. He's not interested in any more milk. And he doesn't like bottles...

I really hope it's not going to be a struggle with solids (I've already had one of those and it wasn't fun!). Look into Baby-Led Weaning (BLW)! They say it's tizz-free.

Muzza1 - thank you for sharing - it's nice to know I'm not alone in my paranoid state! The pead was happy at his 4 month needles, she suggested only seeing the midwife for his 6 month one's but I'll be making an appointment with her as well just to check-in and put my mind at ease. J is exactly the same as your LO - once he's full, he's done. He's not a guzzler - will only take exactly what he wants and needs. I really hope it's not going to be a struggle with solids (I've already had one of those and it wasn't fun!). We're still a way off that anyway... Plenty of time to work myself up into a tizz about that ;) M&S - good to see everyone underneath their cool exterior is as mad as me at times :D

A whopping 130g weight gain!!! :) I'll take what I can get... Will continue the weekly weighs and we'll be seeing the pead in just over two weeks anyway for 6 month needles. Thanks for your support for a super-paranoid mummy that constantly looks for things to worry about! :) That's great news! Of course you'll be paranoid, it's part of the job at times....

Not sure if it's any help hun but it sounds to me like he's doing well - esp if he's a contented little man but always check in with the paed. Emily (also prem) is very slight - almost wirey and lean - and is never going to be big as a baby - like yours very fine featured. At one point she went to 0.4 per cent on the charts and I was panicking but she has climbed (not a lot) but enough I hope. At 6 months corrected (9 months) my paed said she should put on 500 grams per month (I think ?? this is more than a term baby). Well, she's just not doing that...she's put on 400 grams each month for the past 2 months but is growing and is happy (bar a horrendous patch with a cold when forget food she just went big time for the boob which helped her get over it). I am also dreading the weigh in a bit next month - I think she's fine but I know she's not going to tip the scales. She is feeding well but decides when she's had enough and that's it. Re head circumference - she's high on the charts for that and length so looks like longlegged Lionel! And I believe that's normal for prems so they can look a little top heavy. I think everything everyone has said is helpful but I just wanted to let you know I share your anxiety - am dreading the next weigh in but always try to put a brave face on it!! Keep at it - he's a lucky little boy. xxx

I'd probably take guiness's advice and go and see Dr Z, if only for reassurance. From what you've written it all sounds good. i think normal weight gain at 3 months is between 120 and 250g a week. The main thing is wet/ dirty nappies and growth. It's so hard when you've got an extra worrisome baby to figure out which bit to worry about!

You might try taking him to see Dr. Zuhair at Emirates Hospital (Jumeirah and Marina). He is an expert in the growth and development of premature and low birth weight babies. I take my twins to see him and he's been great.

m&s - he's still within the actual scale, just dropped from one range to the lower one. The last time we went for a formal weigh and needles the midwife told me that according to her thinking and the way he was weighing he was never going to be a big buba. He's got quite fine features and will be a slimmer build. If I hadn't weighed him I wouldn't be concerned at all! He's feeding well, lots of wet nappie's, one or two dirty one's a day, generally very happy, sleeps ok (not fab, but he's got two big brother's that wake him up), growing out of his clothe's, and getting lovely little creases in his legs. Just being a paranoid mummy :) Just out of curiosity tho - what is the acceptable range of weigh gain week-to-week for babies in his age range (i.e. 3 months old)?

I used Infinity Clinic for my youngest. There is a lovely lady called Helen, I think she is a qualified health visitor from the UK. She was so supportive and knew her stuff, I had an underweight baby and took her there regularly for checks and advice. She runs the well baby clinic there. Meals. Are you saying that he's dropped below out of the percentile range? If he's skirted along the edge of it for a while he may be nothing to worry about at all (DD dropped 45 places!) but being on the edge of the usual range is a little concerning. What does your paed say about his weight? Is it consistent with his height and head circ? It might be usual for premies to stay on the lower side for a while? Sounds like you're feeding really well so there probably is no cause to worry at all. These rugrats just like to keep us on our toes!

They never go in an exact straight line. We have been charting our baby for 9 months and it is a very wiggly line. the overall trend is what is important and babies have little growth spurts and then rest periods all the time. He could have been having a growth spurt last tie he was weighed. When they are night feeding they tend not to take such a big morning feed, wouldn't worry if he is happy and alert most of the time.

That sounds like a good plan. It sounds as if all is absolutely fine, but it's always a good idea just to keep an eye on things with preemies, and another weighing will give you a clearer idea of what's going on. xx

Thank you HAK and BFC - It's put my mind at ease (marginally) and helped me formulate a plan. I'll take him back next week to weigh him. I did go to a different clinic today as it was closer and easier. The nurse who weighed him said nothing, just read off the weight. Perhaps I should have weighed him after a feed to add that extra 120g ;) I only weighed him a month ago so will up the weekly weighs in the meantime. He's been consistantly floating along the 2nd percentile line (just above it) and only dipped just below it (so not a full percentile drop). With the WHO chart - I was using the normal one for boys just working on his corrected ago (the preemie one is finished for his age now). What comforts me is that he IS a really happy baby. His hunger and sleep cues are very clear and easily sorted. At least 6-7 well wet nappies (very pale color), one or two dirty nappy's a day. The only thing that is worrying/ annoying is that the first feed of the day (around 6:30am) he's not overly hungry - it's a struggle to feed him as he's not really interested in taking much. I wait until he's fully awake and showing signs of wanting to feed but I think he probably takes enough overnight (2 feeds) that he's getting enough. He then has a massive feed at around 9:30-10am to compensate.

Slight variation up and down is normal - it's the overall pattern which is significant. Who did the weighing, and what did they say about his weight pattern? Was he weighed at the same time of day and on the same scales? Is it just this one weighing that has dipped, or has he been showing a gradual decline over some time? Are you using the new(ish) WHO charts for low birthweight babies? Is he as happy and contented as he's always been? Cue fed and allowed to come off one side of his own accord and then offered the other side? Producing plenty of heavy wet pale-coloured nappies? If the answer is yes to all of these then it's likely that all is well. However, if you're at all concerned, then a consult with your pediatrician or an IBCLC should reassure you that all is well (or identify a problem if there is one).

to be honest, I would leave it. Their weight fluctuates, just as ours does. Weigh him again in a week and see where the average line takes him. As you said, he's feeding well and if he isn't interested in any more milk, then he's having enough. Are you eating enough as well though? You should be aiming for about 500 calories more than your usual diet every day and under no circumstances less than 1800 calories/day (I was eating anything between 2300-2500/day when mine were exclusively breastfed) ... and, you never know what's around the corner in terms of growth spurt!
