The clinic i went to only cost 75 aed. I don;t know the exact address but it's just down the street from the crown plaza hotel, where Spice Island is located. They are open till midnight too which is helpful.
There are many workers' clinics in Al Qusais and I think there are some in Karama too. You'd have to drive around to find one, but if you would like the name and number of one specific one, let me know and I'll try to find it out for you.
IMHO, it is not worth it to use them. Firstly, what I wanted to point out is that you don't actually have to do anything to get a health card for your maid. She automatically gets a number when you do her medical and that is enough to take her to the government clinics, if need be.
If your maid is sick, you can just take her to a doctor at one of the affordable clinics around the city. It costs about 100 to see a GP at one of these. I wouldn't even bother with the Iranian Hospital as it is so busy there. The cost of the prescriptions is the same all over so IMHO, you are better off taking your maid to a private clinic.
I bought a medical card for my last maid and renewed it for the 4 years she was with us. The only time she was sick, I took her to the clinic we got it from only to be told that it was now very expensive to come here. Quicker and cheaper to go to the Iranian hospital which we did as they were so insistant!!!
Made me wonder what was the point of the card. I thought it covered emergency care etc.
Now I have a new maid, I am wondering whether to get her a medical card or not. Do you have them ? Have you been able to use it or they just directing all sick maids to the Iranian hospital???
