Can someone please give me a format for maids salary slip which can be used as formal document to maintain maids monthly salary handed over. Need urgently.

we were given a salary card from the agency and she signs that .. My previous maids ... I just printed it off everymonth and it was like this : I ....... confirmed that I have recived my salary for the month of ...... year ..... the amount paid to me is ........... signed ........ ( employee ) signed ..........( sponsor ) I would just change the day every month and keep the papers in the file with her passport Thanks Mushypeas.... this sounds good enuf
