Meniscus Tear and chondromalacia patella - anyone suffering from these?

Hi ladies,

I am new to this forum and really need some advise. Please help.

I'm 36 years old. About 6 weeks ago i started getting mild pain around both knees especially when going up the stairs etc. With certain movements there is also a clicking sound in the knees. I have a very sedentary lifestyle with 0 activity so not sure how I started getting the pain. I went to the docs, especially as my mum suffers from arthritis and osteoporosis. I was told to do a xray and the doc did some general checks and diagnosed me with the above. Very worried and scared as the pain seems to be getting worse. I have a desk job and if i sit for too long, my knees get stiff and the pain gets worse by the end of the day. Also the AC (cold temperature) seems to cause pain in my knees - not sure how or why.. :(

He recommended me physiotherapy but as I was not happy with the hospital or the doctor, Im scared to go to back there or to the wrong people for physiotherapy.

I was also advised to have a MRI by others as they xray does not give full extent of the damage.

*Does anyone suffer from the above and can recommend me a good, well qualified physio and doc (ideally trained in the west) who specialises in this area.
*Im scared of the effects of MRI scans - should I go for this scan?
*Anything I should avoid (exercise) or eat and also any vitamins supplements that can help
*If your medical insurance covers this, how many physiotherapy sessions does it cover? Mine is apparently only 6 and apparently I will need more - minimum 2 times a week is what is recommended to me.
*I have started exercising as the dr. said that i need to strengthen my leg muscles. I dont know what to avoid in the gym - swimming is meant to be the best but I dont have access to a pool. If there are any good websites that can tell me what to avoid and what to do in the gym it would really help. I have googled but keep getting varying opinions.

Ive also had blood tests to include vitamin d and d3 and waiting for results. Fingers crossed!

Sorry I have lots of questions, but i am so worried - Hope to hear from someone soon.

a good phsiotherapist can do tests on your knee to diagnose the exact problem. therefore you can start physio before the MRI. I see Laura at Up and Running on al wasl road. she is from the UK and very good. they have a website so you can read about them.

I have had chondomalacia patella and plica syndrome which is similar to a meniscus tear. I had surgery as a teenager which didn't really improve things and because my knee was immobilised for around 6 weeks there was a lot of muscle degeneration. I needed weeks of physio but after than I curtailed a lot of my exercise. Over the years I just learned to live with the creaking and occasional discomfort but around 5 years ago I was doing a lot of running and I decided to see if I could get it sorted out properly so I went to a sports orthopaedic surgeon in UK (very famous for footballers ACLs.) He did an arthroscopy and cleaned it out. It was a very interesting procedure as I opted to only have a local anaesthetic and he talked me through it and I watched what he was doing on a monitor. (Psychologically it felt good as I could see the gunge he was taking away!) He said it wouldn't be perfect as I am 50+ and there is now a bit of arthritis) but it was much better than before. I did loads of physio with the Scottish Rugby physiotherapist and a PT (daily) and within 6 weeks I did a 30 mile cycle. When my fitness drops or my weight goes up I feel it on my knees so you need to build up all the muscles around your knees to keep them in good working order. Sorry I can't recommend a physio here but when you find one, he/she should be able to plan a programme for you to work on either in the gym or at home. Therabands can be used in a variety of ways and are very effective. Hope you find a way to improve things. Take care.

Would highly recommend Dr Kazim at orthosports. I went there initially in minor pain and he sent me for an MRI. After looking at results suggested i bear with it as long as possible as after being operated on it will bring on arthritus. Went back about 4 years later and had the surgery done. Explained everything very well. Said had I been younger could have been repaired but as older he could only tidy it up!! I felt he wasnt money grabbing and really happy with the results of the key hole surgery. Good luck

Dr. Karim Attara in Al Zahra Medical Centre SZR ..he is excellent, I broke my elbow to pieces 5 years ago and all the doctors said I will not be able to move my elbow full length ever !! .. DR. Karim made it happen. He's the kind of doctor that takes time to explain your condition and answer any questions you have. I highly recommend him ..go see him and take your MRI scans with you ..you need to take an appiontment beforehand because he's always booked..the number is 04 3315000 He's also well known for his miracle surgeries .. he will honestly tell you if a surgery is a must in your case. wotsit , I have the same exact condition which I just found out about a week ago. mine is only in one knee.. I did MRI and the doc confirmed the condition.. however he said it's reversable. I'm an active person and go to the gym daily ..I used to do step classes sometimes but he advised agaist it at the moment .. he didn't recomend any physiotherapy though .. he prescribed a vitamin specailly for joints (can't recall it's name now, I'll post it later) , an anti- inflamatary and pain releif creme Follow your Dr's recommendation and try to be more active ..take a walk for 30 mins a day if you can .. and wear good supported shoes while walking. If you haven't done the MRI ..do it .. it's painless. I'd be happy to hear your progress ..my next check up is next month Get well soon :) edited by Tinker bell on 22/05/2011 Hi Tinkerbell, Thank you for your wishes and I hope that you recover soon from this..I have finally had my MRI and had the results - It was confirmed that I have the issue in one knee and the other knee seems to also suffer from pain but the dr wasnt very clear in explaining. I have been advised on physio, but apparently it will not get rid off the problem and apparently the tear is only reversible through surgery - i have been advised on arthroscopy (spelling?)..Im not so happy with the Dr. so Im getting a 2nd opinion and will take it from there. But im glad to know that its reversible. I am planning to join water aerobics as its suppose to be the best way to reduce the weigth and tone without adding pressure on the joints etc. By the way, which dr are you going to? would also love to keep in touch and see how your progress is . If you have a very sedentary lifestyle you would be so much better off doing some exercise. Plus a healthy diet. Just Google what foods will help heal your body. Instead of worrying yourself and being scared try and help yourself.

BUMP - I am not able to get a MRI scan until a few weeks due to insurance issues so ur advise on the below will highly be appreciated as it will keep me going until then as I cant have physio before that. Could any of you recommend any specific advise that was given to you on vitamins and calcium for the condition? I was told about glucosamine chondritin and fish oils but again not by the dr as he didnt have the time to give me a proper consultation? Also any recommendations on exercise charts and dos and donts available on any websites to help heal the tear?

Hi Ladies, Thanks so much..i feel better already.. I will definitely go for the MRI and then take it from there as one of you mentioned that no point goin to physio until the right diagnosis is available. My dd has a meniscus tear at the moment. We go to the Sports doc at Medicentres Motor City. He sent her for an MRI to confirm his diagnosis. He said she was too young for physio and gave her some exercises to do at home. He said to avoid heavy impact exercises but exercises like walking, swimming & cycling would strengthen the muscles around the knee and help stabilise it and that the knee should heal itself by 3-6 months. Please don't worry, I think it's quite a common injury and surgery is a last result.

Go to Ortho Sports on Beach Road and see Dr Kazim. They have physios etc there. He is VERY experienced in this and will only do surgery as a last resort after trying other things - unlike another doctor mentioned here who suggested radical ( and unnecessary surgery) on a first visit to him. [b'> To add, it all depends on where the tear actually is [/b'> to what they can do. It is important that this is recognised as physio etc all hinges around that. ( personal experience) The number of physio sessions varies as well. Each case is SO individual. It is about 450 to see the doctor and about 275 - 300 for a physio session. It is imperative that you get this addressed correctly as it is your future health that should concern you as well ie to ensure that you have a pain free and active future. did you go to an ortho Dr.? if not, give Dr. Chris Whatley a go at Med Care Hosp. He will also recommend a good phsio. There is also Dr. Schmidt at American Hosp..when my husband had his accident last year and extensive surgery on his knee and thigh this is the guy who did and he was wonderful. Not so much bedside manner but knows his stuff (as does Dr. Whately) There is also a good Phsio dept at the American.

Don't be afraid of an MRI, if it's recommended and can get you an accurate diagnosis, then have it done.
