maid contract question

Hi there
We are in the process of sponsoring a maid and I have a question about the contract - is this a set document that we get from Dubai immigration or the maid's consulate or is it something between sponsor and maid? Seems a silly question but i am not getting clear answers from immigration or the consulate and have sent poor dh to both a number of times to do other things only to ind out we have missed something in the process out!
